Our commitment

PSSC Plastic Surgery Skin Clinic (“PSSC”, “we”, “us”) is a physician-led clinic. We take the privacy of your personal information seriously. As a health care provider in Ontario, we follow the Personal Health Information Protection Act, 2004 (PHIPA), the guidance of the College of Physicians and Surgeons of Ontario on patient privacy, and applicable federal privacy and anti-spam law.

This policy explains what information we collect through our website, online forms, online booking and text messages, how we use it, and the choices you have.

What we collect

When you use our website or contact us, we may collect:

  • Information you give us: your name, email address, phone number, the concern or service you are interested in, and any message or details you choose to include in our consultation request form or question form.
  • Booking and payment information: when you book online, the appointment details and the payment information needed to hold your appointment, such as a credit card authorization. This is handled by our online booking provider, Cortico, using secure systems. Card details are not stored on our website or at our clinic.
  • Information from your visit to our website: your browser, device, pages viewed, and how you arrived at our site (for example, from a Google search or ad). This is collected through cookies and similar technologies.
  • Information from your care: if you become a patient, your health information is collected and kept in your medical record, which is covered by PHIPA.

Please do not include sensitive medical details in our general website forms or in text messages. You can share photos and reports through our secure patient portal after you request a consultation.

How we use your information

We use your information to:

  • respond to your consultation request or question;
  • help you book, confirm and prepare for an appointment;
  • send you reminders and follow-up messages about your request, by phone, email or text message;
  • provide health care, once you are a patient;
  • send you news and updates, only if you have chosen to receive them;
  • understand how our website and advertising are performing, and improve them; and
  • meet our legal and professional obligations.

We do not sell your personal information.

Text messages

If you give us your mobile number on our request form and tick the box agreeing to receive text messages, you agree to receive text messages from Dr. Hayward’s office about your consultation request. For example, we may text you a link to book your consultation, a reminder to complete your booking, or a notice about a change to our booking calendar.

  • How to stop: reply STOP to any text message and we will stop texting you. Reply HELP for help, or call us at 905-898-1814.
  • Frequency: message frequency varies and is tied to your request, typically 1 to 4 messages.
  • Cost: message and data rates may apply, depending on your mobile plan.
  • Sharing: we do not share mobile information, including your phone number and your text-message opt-in and consent, with third parties or affiliates for marketing or promotional purposes. We do not sell or rent it. Text messages are sent through our messaging service provider, Twilio, which handles them on our behalf only.
  • Consent is not a condition of care: you do not have to agree to text messages to receive care. You can call us at 905-898-1814.

Who we share information with

We share information only as needed to run our clinic, and only with service providers who help us do so, including:

  • our website form provider (Formspree), which delivers your form submissions to us;
  • our online booking, payment processing and secure patient portal provider (Cortico), which uses Square to process payments;
  • our messaging provider (Twilio) and our email provider, which send messages on our behalf;
  • website analytics and advertising measurement services (Google Analytics and Google Ads); and
  • our practice management and electronic medical record systems.

We may also disclose information where you have consented, where PHIPA or other law permits or requires it, or to protect someone’s health and safety.

Some of these providers store information on servers outside Canada, including in the United States. Where that happens, the information may be subject to the laws of that country.

Cookies and tracking

Our website uses cookies and similar tools, including Google Analytics and Google Ads measurement, to understand how visitors find and use our site and how our advertising performs. You can control or delete cookies in your browser settings. You can also opt out of Google’s measurement through Google’s tools.

Email

If you ask to receive news or updates, we will email you. You can unsubscribe at any time using the link in any email or by contacting us. We will still send you messages that are needed for your care or your request.

Keeping your information safe

We use reasonable safeguards to protect your information from loss, theft and unauthorized access, including secure online systems and limiting access to the staff who need it. No method of sending information over the internet is completely secure, so please use our secure patient portal for anything sensitive.

Keeping your information

We keep personal information only as long as we need it for the reasons above or as required by law and professional standards. Medical records are kept for the period required by the College of Physicians and Surgeons of Ontario and PHIPA.

Your rights

You can ask to see the personal information we hold about you, ask us to correct it, or withdraw your consent to certain uses, subject to legal limits. If you are concerned about how we handled your information, contact us first. You can also contact the Information and Privacy Commissioner of Ontario.
